Form Schedule Best Practices

This week’s tip takes us back to the basics of form filing. All filers will benefit from tips outlining best practices for completing the Form Schedule in SERFF.  

  • Uniform Standards require that the form contain an unique form number in the lower left hand corner of the form and in the format prescribed by the Uniform Standards.  The prefix of the form number is to contain "ICCxx" where "xx" is the year the product was submitted for filing.
  • Before submitting your filing in SERFF, double-check that the form number in the Form Schedule column matches the number on the form itself (as well the number on the Statement of Variability and the number on the Actuarial Memorandum, if applicable).
  • Make sure the documents attached to the Form Schedule do not contain tracking or revision marks—only clean versions can be approved. 
  • All forms should be in a searchable PDF format.
